Waiver of Responsibility:

 

You understand and acknowledge that you have consulted your healthcare provider regarding elective ultrasounds. Bella 3D Imaging provides non-diagnostic, keepsake services for personal leisure only. This session does not replace professional prenatal care, and Bella 3D assumes no responsibility for detecting or reporting pregnancy complications to you or your OB. Although our technologists are registered and highly qualified, they are NOT physicians and they are NOT authorized to diagnose any medical conditions. They can offer their educated opinion, but you agree to follow up with your OB for all medical concerns and diagnostic needs.

 

You understand and acknowledge that sonograms can be limited due to many variables such as amount of fluid, fetal positioning, and maternal body habitus.  Because of this, pictures of your baby are not a guarantee. In the event this occurs, Bella 3D Imaging provides a courtesy one-time follow up visit that has to be utilized within the first two weeks of your initial appointment. Pregnancy confirmations, packages purchased at a discounted rate, or monthly promotions are NOT eligible for a re-scan. You agree that all sales are final.

 

You understand and acknowledge that Bella 3D Imaging cannot accommodate late arrivals. Arriving late to your scheduled reservation will be deducted from your session and this missed time cannot be extended. Cancellations are non-refundable, however if you cancel your session 24 hours or more in advance, we will issue a full refund. If you need to reschedule on the day of, there will be an additional $50 scheduling fee.

 

To ensure the efficiency of our session times and best serve all of our clients, full payment is required to confirm your booking. Payment can be made with Zelle/Venmo. This policy allows us to dedicate our full resources to your scheduled session and minimize scheduling conflicts. If we do not hear back from you OR payment is not received within 24 hours of receiving our email after a request is made, your reservation will automatically be declined. 

 

By agreeing to these terms and conditions, you are willing to comply and proceed with scheduling your session with Bella 3D Imaging.
